The National Transportation Safety Board (NTSB) has intensified its scrutiny of Level 2 semi-autonomous driving systems, particularly after two fatal incidents involving Ford Mustang Mach-E vehicles using the BlueCruise system. The crashes occurred in 2024 in San Antonio, Texas, and Philadelphia, Pennsylvania, leading to the deaths of three individuals. Both vehicles were reportedly using the BlueCruise system at highway speeds when they collided with stationary objects.

According to NTSB reports, the investigation revealed that neither vehicle executed any braking or steering maneuvers prior to impact, raising alarms about the potential overreliance on such systems. The NTSB’s findings highlight critical flaws in driver monitoring capabilities, which failed to detect distractions like phone use.

Details of the Crashes and System Failures

In the Texas incident, a 2022 Ford Mustang Mach-E struck a stationary 1999 Honda CR-V at around 75 mph, resulting in the death of the Honda driver. Similarly, the Philadelphia crash involved a Mach-E colliding with a stationary Hyundai Elantra and a Toyota Prius, killing both drivers. Reports indicated that the driver of the Mach-E in Philadelphia was holding a cell phone at the time, further complicating the situation.

The NTSB noted that the BlueCruise system’s driver monitoring was insufficient, allowing for significant distractions to go unnoticed. In its analysis, the board found that the system’s adaptive cruise control could enable speeds up to 20 mph over the legal limit, exacerbating the severity of collisions.

Regulatory Gaps and Recommendations

NTSB Chairwoman Jennifer Homendy emphasized the urgent need for enhanced federal standards governing Level 2 systems. Currently, there are no federal requirements mandating that manufacturers collect crash data for vehicles operating in semi-autonomous modes, leaving investigators without crucial information. “We cannot take a ‘hands-off’ approach to hands-free driving technology,” Homendy insisted during a public board meeting on March 31, 2026.

Among the NTSB’s recommendations were calls for the National Highway Traffic Safety Administration (NHTSA) to establish performance standards for Level 2 systems. This includes mandatory crash data recording and enhanced driver monitoring systems to better identify distractions.

Operational Risks and Industry Implications

The BlueCruise system has been criticized for permitting the disabling of automatic emergency braking while in hands-free mode. This feature, coupled with the allowance for excessive speeding, raises significant safety concerns. NTSB board members described the current state of Level 2 automation as the “Wild West,” indicating that regulatory oversight is lagging behind technological advancements.

With the NTSB’s findings, manufacturers like Ford may face increased pressure to revise their systems. The investigation sheds light on systemic issues in the automotive industry regarding the deployment of semi-autonomous technologies, urging a reevaluation of practices that prioritize safety.
